Add Constituent Financial Institution Screen

Screen Item	Description
Financial institution	Enter the name of the financial institution.
Branch	As financial institutions often have multiple branches, enter a name to identify this branch of the institution, such as Downtown or Baker Street.
Routing number	Enter the routing number used to identify the financial institution.
Rejection code	Enter the rejection codes associated with the routing number. Altru uses this information to determine whether the financial institution rejects a direct debit transaction.
Contact information	Enter the address and telephone number information for the financial institution.
