Learning Profile Forms

Learning profile forms are personalized forms for student development. At most schools, these replace paper Individualized Education Program (IEP) forms. When creating learning profile forms, you can choose between building a learning profile form template within the system or creating a file upload form that allows the upload of external files to use as learning profile forms.

You must be logged in as a user with the Learning Profile Manager role to access the following settings.

Add a Learning Profile Form

  1. In Academics, select Learning center.

  2. Select Learning profile forms.

  3. Select +New form, then select the form type:

    • Template: build your form template within the system.

    • File: upload external files when adding this form to individual student profiles.

Create a Learning Profile Form with a Template

The form template setup is divided into three sections:

If using an external template, you can set up a learning profile form for uploading files to individual students that serve as their learning profile forms.

When a file is uploaded as a student's learning profile form, you will upload an external file instead of completing a form template within the system. The uploaded file will be encrypted.

When teachers or other users generate a learning profile form for the student from rosters or Academic profiles, the uploaded file is displayed.

Enter the following information for the file upload profile form:

  1. Learning profile name: Name for the learning profile form, e.g., Individual Education Plan.

  2. Approval required?: Select this if you want a manager to approve the form before it is posted.

  3. Active: Disable only if you want to deactivate the form.

  4. Sort order: Order that the form will appear in a drop down list of forms. Smaller numbers appear higher on the list.

  5. Select the roles that can access the form:

    • Report: Shows a student's learning profile to users with the role.

    • Icon View: Places a learning profile icon next to the student's name in the roster channel on the class section pages.

Edit a Learning Profile Form

  1. In Academics, select Learning center, then Learning profile forms.

  2. Select the context menu next to the form, then Edit.

  3. Make any necessary changes and select Save or Save & Exit.

Run Learning Profile Reports

Once your school has created learning profile forms and filled them out for students, you can generate lists and spreadsheets of students with learning profiles.

  1. In Academics, select Reporting, then Reports.

  2. Select Filter and set the Category filter to Learning Profiles.

  3. Select one of the following reports to run:

    • Learning Profiles - By Author

    • Learning Profiles - By Student

    • Learning Profiles - By Type

    • Student Learning Profiles

  4. The report generator will open in a new browser window. Set your parameters, then select Generate Report or choose one of the file export options.
