| Report Column | Description |

| Constituent Name | Last and first name of each constituent who clicked the Unsubscribe link in the email message |
| Email address of each constituent who clicked the Unsubscribe link in the email message | |
| Message Name | Name of the message from which the constituent responded |
| Email Subscription Change | Name of the option that the constituent selected from the Unsubscribe dialog |
| Email Opt Out Date | Date time stamp when the constituent unsubscribed |
