Donation Form Templates

The Online Giving application includes several templates that you can use directly out of the box, or as a starting point to create a custom donation form.

All sites have access to the following templates:

• Basic Donation Form that includes the minimum data elements necessary to process a transaction using the Standard Donation Level data element with only a donor-entered giving level. You can add more giving levels. This is a good choice for connecting to a Dynamic Ask component or whenever a very basic form is required.

• Basic Donation Form with Suggested Levels that includes the same minimum data elements required as the Basic Donation Form and includes three preconfigured giving levels and a donor-entered level. This is an excellent choice for the majority of your donation form needs since you can easily modify the pre-configured giving levels with amounts of your own choosing or unpublish levels you don’t need.

• Honor Gift Donation Form that is similar to the Basic Donation Form with Suggested Levels except that this form allows donors to honor someone through a memorial or honor gift and to request that your organization forward a personal note to the honoree or other notification recipient. If your site is configured to offer recurring gifts, this template also offers donors the option of repeating their gift on a regular basis (although you can disable that feature if you choose. The honor gift notification information will be available to you through the downloadable version of the Donations by Transaction report. This template is a good choice if you would like to offer your donors the opportunity of giving in honor or in memory of someone else.

• Matching Gift Donation Form that provides the ability for donors to enter or search for their employing company to initiate a matching gift if your organization is contracted with HEP.

In addition, if your site is configured to offer:

• Premiums, the Donation Form with Premium Selector template will be available. This form is similar to the Basic Donation Form with Suggested Levels except that it includes a Premium Selector to enable donors to select products and services as premiums with a giving level. This form also includes Shipping Fields to fulfill most premium requests. Note that this template does not include a Donor-entered giving level, since premiums cannot be offered with that type of level.

• Designated Giving, the Designated Giving Donation Form template will be available. This form has Designated Giving Donation Level element to allow donors to select specific designees that have been created in advance to receive their gifts. You can also add a write-in field.

• eCards, the Honor Gift Donation Form with eCards template will be available. This form is identical to the Honor Gift Donation Form mentioned above, except that the donor is offered the opportunity to send an electronic card directly to recipients rather than requesting that your organization forward a note. This form requires that you configure at least one Stationery for Donation eCards for your site.

• Memberships, the Membership Donation Form will be available. This form allows donors to receive a membership benefit with a giving level. The administrator controls the membership type that will be associated with each giving level. The membership types must be created before using this form.

• TeamRaiser events, the TeamRaiser Donation Form template will be available. This form allows a donor to contribute to a TeamRaiser event, team, or participant at one of four giving levels, including a donor-entered level. It also allows donors to make gifts anonymously or specify the name they would like used on the TeamRaiser honor roll and include personal notes to participants.

• Personal Fundraising campaigns, the Personal Fundraising Donation Form template will be available. This form allows a donor to contribute to a Memorial, Honorary, or Personal Fund in a Personal Fundraising campaign. It allows donors to make their donations anonymously or to specify the name that they would like used for recognition. The template has three preconfigured giving levels plus a donor-entered level available.

• Recurring (or sustaining) gifts, the:

  • Monthly Giving Donation Form template will be available. This form is similar to the Basic Donation Form with Suggested Levels except that each giving level is configured to provide for a specific repeating pattern and amount. The template is set up to offer monthly repeating behavior that is ongoing (no set end date), but you can modify that behavior so that the repeat is quarterly or annually or for a set period of time.

  • Flexible Sustainer Donation Form template will be available. This form uses the Flexible Sustainer Donation Level element and allows donors to design their own recurring gift patterns, chosen from the options. For example, if using the template exactly as configured, the donor might choose a giving level of $35 and then decide to repeat this gift monthly for 6, 8, or 12 months. The form also supports one-time gifts. This template is a good choice if you would like to offer your donors a variety of sustaining options.

  • Installment Payment Donation Form template will be available. This form uses the Installment Donation Level Element and encourages donors to commit to a high-dollar gift by allowing the gift to be paid in installments. For example, if using the form exactly as configured in the template, the donor might choose to give $2000 and spread that out over 6 monthly payments.
