Add Custom Fields to Constituent Forms
To collect information about the form submitter, such as how they would like to volunteer, add constituent custom fields to your constituent forms.
Before you can add a custom field to a form, it must exist as a constituent field in Settings, Custom fields. Fields set to only allow one per record will overwrite existing data. For more information, see Custom Fields.
The types of fields available for constituent forms are:

Text
-
Number
-
Date
-
Yes/No
Tip: The Yes/No field is a checkbox. When a submitter leaves it unchecked on a form, a "No" response is recorded.

Note: You can add a field only once to a form, but you can add it to multiple forms, change its label, and choose whether a response is required.
To add a constituent custom field to your form:

From Online Giving, select Constituent forms.

To create a new form, select New form.

To edit a form, from the form's ellipsis, select Edit.

From Form, select and move Custom fields from Fields in the Form designer to a location on the form.

Search for the field by name and select it.

To change the field label, update the Custom field label.

To require a response from submitters, select Response required.

Select Done.

Continue configuring the form or save it.
