Add Tribute to Donation Forms
With the Honor/Memorial Tracking module, you can give donors an opportunity to dedicate their gift to a person or cause on donation forms. Recipients are also known as acknowledgees in the database view and web view. They are individuals — such as the honoree or someone close to the honoree — donors choose to notify about their gifts.
To add tributes to your forms, from Online Giving, Donation forms, create or edit a form.

From 1. Donation, select Enable donors to dedicate their gift to a person or cause under Tribute gifts.

To select the tribute types donors can choose from, begin entering a tribute type and select it. Continue for each tribute type you would like on the form.

Select whether to collect information from donors based on how your organization will notify recipients.
Note: To enable you to send communication about a tribute to recipients, you can collect their email and mailing addresses.

Continue configuring the form or save it.

From 3. Form, select and move Tribute from Fields in the Form designer.

From Conditions:

Select whether a tribute is required to make a gift on the form and the text that displays when it's optional.

Select the tribute types available on the form.
Note: To add more tribute types, from Settings, Tables, select Tribute Types.

From Notifications, select whether to collect information from donors based on how your organization will notify recipients.

From Labels, you can update the labels for checkboxes and fields for tributes.
Tip: As you make changes, you can see them in the Preview.

Select Done.
