Questionnaires
Use questionnaires to ask donors qualifying questions for your company's match programs. Depending on your company settings, donors can request a company match to their donation:

During the donation process.

After the donation is submitted and any approvals are complete in My Giving, Match Programs, Donated YTD.
To display a questionnaire during the checkout flow, admins must add it to the match program's settings.

Navigate to the CSRconnect Admin Portal, select Give Settings from the left navigation bar, and then select Questionnaires.

Enter the Questionnaire Name.

Select Active if the questionnaire should be active.

Select Add Question for as many questions as you want for your questionnaire. Fill out all necessary fields. Select the delete icon next to a question if you want to remove it.

Select Save.

Navigate to the CSRconnect Admin Portal, select Give Settings from the left navigation bar, and then select Questionnaires.

Locate the questionnaire you want to edit.

Select Edit set.

Include any changes as needed.

Select Save.
