Alert Question Response Report
Custom survey-type questions for an Action Alert or a Letter to the Editor Alert are created on the Include Questions process page. To view a report of advocate responses to the custom questions in an alert, do one of the following:

Navigate to the Alert List page. In the Actions column, click Download Report to generate a new report for the alert. (The link does not display for preexisting older alerts without custom questions.) The Report Results page opens, from which you can view, download, or print the current or previous reports. If the alert has no custom questions, the report will be empty.

(Action Alerts only) Navigate to the Include Questions process page of your alert. Under Related Actions at the bottom of the left pane, click on View Question Responses. You can review answers to your custom questions, evaluate trends, and so on.

These question response reports can be accessed only from their associated alerts. They cannot be accessed from Reports or Surveys. (If you configure a custom question to update a field in the constituent Profile in Constituent360, individual responses can be viewed in the Profiles of constituents who complete the question.)
