Check boxes can be added as a group or as a single option. You can also add a text field to a group of check boxes to allow someone to enter custom data. For example, you could add a check box called Other, and then place a text field next to it where someone could enter information.
Note: You can edit the properties of a group (such as the group label) from the dialog of any single option within the group.
