| FIRST NAME | First name of the intended recipient |
| LAST NAME | Last name of the intended recipient |
| STREET | Number and name of the street of the intended recipient |
| CITY | Name of the city in which the intended recipient resides |
| STATEPROV | Two-letter abbreviation of the state or name of the province in which the intended recipient resides |
| ZIP | Postal ZIP code in which the intended recipient resides |
| COUNTRY | Name of the country in which the intended recipient resides |
| PHONE | Telephone number of the intended recipient |
