Multiple Choice Multiple Response Questions
Use the Multiple Choice Multiple Response question type to gather more than one and up to a maximum of preferred choices from several options.
If the appropriate site option is enabled for this type of question, you can configure each response to populate a field in the constituent record of each participant. For example, if you want to gather information about cancer survivorship, you can create SelfSurvivor, FamilySurvivor, and FriendSurvivor as custom Additional Information fields in the Profile and then configure the Multiple Choice Multiple Response Question to add Yes to each custom field for a respondent who clicks the corresponding checkbox or No if they do not click it.
Note: Your site has only 30 custom string fields to work with in the Profile so it would be wise to consider the importance of using the fields for this purpose. All fields of a string type will appear in the option box, so you must be careful when specifying the field to update so that you do not over-write important data.

Respondents can click the choices they prefer. If the question is marked as Required and the minimum and maximum number of choices is set, respondents MUST click the minimum number of choices to submit the page containing this question.
