Quick Reference - Include Questions Page
The Include Questions process page includes the following fields, actions, and columns:
Field/Action/Column | Description |

Add a Question | Click to create a new custom question. Opens the Edit a Question page. |
Questions | Displays the text of the question. |
Actions |
* Edit link - Click to open the Edit a Question page and edit the question.
* Delete link - Click to delete the question. |
Order | Indicates the display order of the questions. To change the order, edit the numbers in the text boxes. The questions themselves are not numbered when they display on the published page.) |
Question Type | Indicates the Question Type, such as Date Value or Multiple Choice Combo Box. |
Update Constituent | Yes/No indicates whether a field in the constituent Profile is being updated with responses to this question. You can set the Profile to be updated when you configure the question. |
Required | Indicates whether the question is required. If it is, constituents must provide a response to the question in order to send the alert. |
Include this Question | Use these checkboxes to select individual questions to include in the alert:
*Select a checkbox to display that question to constituents. or... * Deselect a checkbox to hide that question; it is still available if you decide to toggle it on again. (You can also delete a question permanently.) |
