Select Grant Program Forms
Use the Forms tab to define the applicant intake experience for a grant program. Most programs use a required Default Form and may also include an optional Eligibility Form to screen applicants before they start the full application.
Note: Program-level form selections are managed in Program Detail > Forms.
How Program Forms Work

Default Form is the primary application form and is required for program setup.

Eligibility Form is optional and can prevent ineligible applicants from proceeding to the Default Form.

Additional forms can be assigned at workflow levels to support review, reporting, and follow-up tasks.
Select Forms for a Program

Open Grant Programs.

Create a program or open an existing one. For step-by-step setup, use Create New Grant Programs or Edit Grant Programs.

Select the Forms tab.

Select the Default Form for the primary application experience.

Optionally select an Eligibility Form and configure matching responses when pre-screening is required.

Save your changes.
Choose the Right Form Strategy
If your goal is broad intake with fewer barriers, use only a Default Form. If your program has strict entry criteria, start with an Eligibility Form to reduce manual review volume and improve applicant quality earlier in the process.

Manage Forms Across Workflow Levels
After selecting program-level forms, configure level-specific requirements in Manage Workflow Level Forms so applicants and grant managers receive the right form at the right stage.
Tip: Before assigning forms, confirm your form assets are ready in Create and Manage Custom Forms to avoid mid-cycle changes.
