Donation Form Templates

The Online Giving application offers several templates that you can use right away or customize to create a unique donation form.

Available Templates for All Sites:

Basic Donation Form: Includes the essential data elements to process a transaction using the Standard Donation Level with a donor-entered giving level. You can add more giving levels. Ideal for connecting to a Dynamic Ask component or when a simple form is needed.

Basic Donation Form with Suggested Levels: Similar to the Basic Donation Form but includes three preconfigured giving levels and a donor-entered level. Easily modify or unpublish levels as needed. Suitable for most donation form needs.

Honor Gift Donation Form: Like the Basic Donation Form with Suggested Levels but allows donors to honor someone through a memorial or honor gift. Includes an option for recurring gifts and honor gift notifications. Great for offering donors the chance to give in honor or memory of someone.

Matching Gift Donation Form: Enables donors to enter or search for their employing company to initiate a matching gift if your organization is contracted with HEP.

Additional Templates Based on Site Configuration:

Donation Form with Premium Selector: Similar to the Basic Donation Form with Suggested Levels but includes a Premium Selector for donors to select products and services as premiums with a giving level. Includes Shipping Fields but does not support donor-entered giving levels.

Designated Giving Donation Form: Allows donors to select specific designees to receive their gifts. Includes a write-in field.

Honor Gift Donation Form with eCards: Identical to the Honor Gift Donation Form but allows donors to send an electronic card directly to recipients. This form requires at least one Stationery for Donation eCards for your site.

Membership Donation Form: Allows donors to receive a membership benefit with a giving level. The administrator controls the membership type associated with each giving level. The membership types must be created before using this form.

TeamRaiser Donation Form: Allows donors to contribute to a TeamRaiser event, team, or participant at one of four giving levels, including a donor-entered level. Supports anonymous gifts and personal notes to participants.

Personal Fundraising Donation Form: Allows donors to contribute to a Memorial, Honorary, or Personal Fund in a Personal Fundraising campaign. Supports anonymous donations and recognition names. Includes three preconfigured giving levels plus a donor-entered level.

Recurring (or Sustaining) Gifts Templates:

Monthly Giving Donation Form: Similar to the Basic Donation Form with Suggested Levels but each giving level is configured for a specific repeating pattern and amount. Set up for ongoing monthly repeats but can be modified for quarterly, annual, or set period repeats.

Flexible Sustainer Donation Form: Uses the Flexible Sustainer Donation Level element, allowing donors to design their own recurring gift patterns. Supports one-time gifts and various sustaining options.

Installment Payment Donation Form: Uses the Installment Donation Level Element, encouraging donors to commit to a high-dollar gift paid in installments. For example, a $2000 gift spread over 6 monthly payments.
