Donation Form Templates

Online Giving includes templates you can use as-is or customize to create donation forms.

Templates available for all sites

Basic

Use the standard donation level with a donor-entered giving level. Add more giving levels as needed. Use this form with a dynamic ask component or when a basic form is required.

Basic with suggested levels

Include three preconfigured giving levels and a donor-entered level. Modify or unpublish levels as needed. Use this form for most donation scenarios.

Honor gift

Include suggested giving levels and let donors give in honor or memory of someone. Support recurring gifts and honor gift notifications.

Matching gift

Let donors enter or search for their employer to initiate a matching gift if your organization contracts with HEP.

Templates based on site configuration

Premium selector

Include suggested giving levels and a premium selector for donors to choose products or services. Includes shipping information. Does not support donor-entered giving levels.

Designated giving

Let donors select specific designees to receive their gifts. Include a write-in option.

Honor gift with eCards

Same as the honor gift donation form but includes eCards. To use donation eCards, create at least one stationery.

Membership

Let donors receive a membership benefit with a giving level. Assign membership types to giving levels. Create membership types before using this form.

TeamRaiser

Let donors give to a TeamRaiser event, team, or participant at one of four giving levels, including a donor-entered level. Support anonymous gifts and personal notes to participants.

Personal fundraising

Let donors give to a memorial, honorary, or personal fund in a personal fundraising campaign. Include three preconfigured giving levels and a donor-entered level. Support anonymous donations and recognition names.

Recurring (or Sustaining) Gifts

Monthly giving

Include suggested giving levels configured for a repeating pattern and amount. Defaults to monthly gifts but can be modified for quarterly, annual, or fixed periods.

Flexible sustainer

Use the flexible sustainer donation level element. Donors design their own recurring gift patterns. Support one-time and sustaining gifts.

Installment payment

Use the installment donation level element. Encourage donors to commit to high-dollar gifts paid in installments.

Note: The difference between sustaining gifts and installment gifts is that sustaining gifts are a commitment to a defined duration of payments. Installment gifts are a commitment to a defined total amount given. Sustaining payments will always be the same amount. Installment payments may vary.
